4 Nutrition tips to Reduce Lymphoedema Risk after Orthopaedic-surgery

Orthopaedic surgery can place significant stress, damage or even remove part of your lymphatic system. Your immune system plays a vital role in maintaining fluid balance and immune defense. However, after orthopaedic or any type of surgery, especially procedures involving lymph node removal or disruption, the lymphatic network may become compromised, leading to fluid accumulation and persistent swelling known as lymphoedema. Among other strategies, proper nutrition is a crucial to reduce lymphoedema risk, yet often overlooked; it is also a key element of post-surgical care that supports lymphatic health, improves and speeds up healing, and reduces the risk of developing other secondary conditions. Inflammation is a natural response to surgery, it means you are in a recovery process, but excessive or prolonged inflammation can worsen lymphatic congestion, edema and turn into lymphoedema (also caused by damage in your lymphatic system). After surgery, your  lymphatic system can be more vulnerable to fluid retention leading to lymphoedema. Eating properly to reduce systemic inflammation and oxidative stress is key to avoiding chronic inflammation. Polyphenols, natural antioxidants found in colorful fruits and vegetables, specifically target inflammation and support healthy lymphatic vessel function. Weight management through a balanced, calorie-controlled diet also improves lymphoedema symptoms and reduces limb swelling by lowering pressure on lymphatic ducts. 1. Eat Foods to Promote Circulation and Reduce Inflammation

Focus on whole, unprocessed foods that contain anti-inflammatory nutrients, antioxidants, and healthy fats, for example:

  • Leafy greens

 Detoxify the body, are full of antioxidants and provide magnesium for circulation and muscle relaxation, allowing lymphatic and vascular flow. Some that you can include are:

      Common Leafy Greens

Spinach, kale, lettuce (romaine, butterhead, iceberg, leaf, etc.), Dandelion,  Swiss chard, collard greens, mustard greens, arugula (rocket), cabbage (green, red, savoy, napa), etc.

 Herbs often used as greens (have same properties)

Parsley, cilantro (coriander leaves), basil, dill, mint.

  • Berries

They contain antioxidants and flavonoids that reduce oxidative stress and inflammation that can block lymph flow. Some of them are: raspberry, blueberry, blackberry, cranberry, gooseberry, etc.

Feeling blue?  Having low mood and sadness are normal responses to difficult situations, those that move something inside us. But what happens when this sensation persists over time? This human experience is linked to an imbalance in the complex neurochemical network in our bodies, particularly involving the neurotransmitter serotonin and the Gut-Brain Axis. For that reason, today, we will talk about: why we feel blue, the role of the multifaceted serotonin and how the gut-brain connection works. Let’s have a closer look…

What Is Serotonin?

Serotonin(link1)( 5-hydroxytryptamine (5-HT)) is a neurotransmitter found throughout your brain. It has a pivotal influence in your:

  • mood 
  • emotion (fear)
  • sleep patterns
  • appetite
  • digestion
  • cognitive functions (memory, focus)
  • stress response
  • sexuality
  • addictions
  • breathing
  • body temperature

Why is serotonin so important for mental health?

Serotonin is your natural mood booster, when hormone levels are balanced and enough, you feel calm, focused, and happy. On the other hand, low levels of it are strongly linked to depression and feelings of deep sadness (“feeling blue”). On the contrary, too high levels of serotonin gives you that sensation of Euphoria.

Why do we feel blue all the time?

This persistent “blue sensation- feel blue”, with no apparent reason, can arise from serotonin imbalances in your brain, affecting mood regulation, stress response, and your cognitive functions (focus, memory, etc.). Factors that influence these imbalances include: genetic predisposition, environmental stressors, diet, and gut microbiome composition (because your gut produces more serotonin than your brain).

When serotonin signaling is disrupted, brain circuits involved in emotion and motivation are affected, leading to symptoms of depression or anxiety. For that reason, enhancing serotonin level is the main target in depression or trauma treatment.

Serotonin outside my brain?: How is it possible?

It might surprise you, but about 90% of our body’s serotonin is produced in the gut;  yes, our intestines produce the majority of the happy hormone! Actually, serotonin is produced by specialised cells called enterochromaffin in the intestinal mucosa thanks to the help of microbiota (good bacteria in your intestinal tract). 

This gut-derived serotonin directly influences appetite and digestion, but does not cross the blood-brain barrier directly. Despite this division in production, your gut’s serotonin plays a crucial role influencing brain function and mood.

How can the brain and gut communicate?: The Gut-Brain Axis

Your gut and brain communicate through a bidirectional system known as the gut-brain axis. This system links the enteric system (nerves in the wall of your gastrointestinal tract), the vagal nerves (part of the autonomic nervous system, connecting CNS and organs) and the central nervous system (CNS brain-spinal cord). This network also includes endocrine, humoral, metabolic, and immune routes. 

How does gut serotonin affect your mood and cognitive function?

The serotonin produced in your gut interacts with the 5-HT receptors (serotonin from the brain) on vagal nerves, then, the signals are sent through the vagus nerve to the brainstem and are received in different brain regions influencing mood, cognitive function, mental health and your general well being.

How can I support my serotonin production?

As we mentioned before the majority of your serotonin is produced in the gut, so when your intestines are full of good bacteria and these organisms are well fed your serotonin levels will be balanced.

Happy intestine, happy brain?

Ingest probiotics (to feed microbiota) through what you eat, for example: kefir, Greek yogourt, fermented food, kimchi, etc., these are good probiotic providers. If your microbiota is low, you can take pills of probiotics, there are different brands, just be sure they have plenty of strains and a lot of millions of each of them. Remember you should have a diet full of good prebiotics and fiber (what probiotics eat). Just eat properly, consume a wide variety of vegetables, fruits, whole grains, legumes, and nuts; drink plenty of water, reduce stress factors, sleep well and avoid processed food and alcohol.

Feeling blue is  normal after some events, what you have to pay attention to is that it does not persist over time or affects your daily life.  If it does, check your serotonin levels, what you eat and your habits. Remember that serotonin produced in the gut supports your brain serotonin, so a happy intestine means a happy brain and vice versa.   • Choose Healthy Fats (rich in Omega3): Incorporate sources like avocado, olive oil, nuts, and seeds while avoiding trans fats found in processed foods.
  • Reduce Salt Intake: Excess salt can lead to water retention in tissues; instead, you can opt for low-sodium alternatives.
  • Stay Hydrated: Drink at least two liters of water daily to support lymphatic function.
  • Avoid Blood Sugar Spikes: Avoid consumption of refined/processed products.
  • Exercise Regularly: Pair dietary changes with physical activity to enhance mobility and manage stress.

Why should I count calories on Christmas?

The average amount of calories you eat on Christmas day are 5,373 (beverages included). This is the twice recommended amount for men and three times for women, so think about how much time you will need to burn out all those extra calories.

Are you having plastic surgery?

If that’s your case, let me tell you my dear friend that you are not alone, in 2019, roughly 27 thousand cosmetic surgeries were carried out in the UK.  When considering cosmetic procedures such as a nose lift or lip enhancement, one of the most  concerns for some people is wound healing, especially if the surgery has been done abroad. What to do if the wound gets infected, how to clean it daily without medical assistance and so on. It is important to take care of the wound in order to speed up the healing process, prevent complications and have the nice results you expect. 

The first step in your wound healing process is to follow your surgeon’s instructions in order to prevent further issues (infections or visible scars). In this blog, I give you 7 tips you can take into account to take care and speed up the wound healing process, let´s know them!

1.Keep your incision clean and dry.

Keep your incision clean and dry all the time, don’t allow water in your wound for the first 24 hours after surgery, or until the plastic surgeon recommends it. 

On the other hand, you have to avoid non-prescribed ointments, creams, lotions or gels in the wound at least 2 weeks after surgery. If there is an opening or a spitting stitch (that’s a separate issue and you need to talk to the surgeon or nurse). Excess of moisture can cause the stitch to fall and open, that is the reason why the incision must be dry. 

If you notice  that your wound takes longer to heal, sometimes this is due to lack of ventilation to the wound. Remove your garment for one or two hours per day and allow the wound to get fresh air, of course talk to your surgeon first.

2.Good energy means eating properly and stay hydrated

Food is your principal source of energy and helps your body to heal properly. However, not all foods are good for you, eat what benefits your body the most, in my blog What to eat to heal faster you can find more information on what to eat and what to avoid.  

Food has different roles for example:

  • increase wound healing
  • anti-inflammatory 
  • boost your immune system
  • create new blood cells
  • reduce swelling
  • heal faster
  • etc.

As food is key for wound healing it is also hydration. Increase your plain or flavored water intake in order to eliminate toxins and residual products from the surgery, but avoid high calorie (sugar added) and carbonated beverages.

3.Clothes matters

What you wear is important for wound healing. Avoid tight and synthetic clothes specially on the incision area because it can cause irritation. In addition, according to your surgery type you will have to wear a compression garment right after surgery. It is key to reduce swelling and edema, increase fluid and lymph flow, help contour the area and protect the incisions from direct rubbing, so look for the correct one that really fits you. Invest in good quality garments to avoid skin and wound irritation (Colombian brands are the best!). 

4.Movement vs Rest 

It is important to move right after surgery. Move as much as you can, but be careful. You can start with walking or pumping exercises, these improve blood flow, prevent blood clots and increase lymph movement. All of them improve and speed up wound healing.

On the other hand, you should always listen to your body. If you feel tired just rest! You may feel anxious about getting back to work, but allow yourself to rest as needed. Ask your surgeon about returning to your daily life activities and follow her/his instructions.

And don’t forget to avoid movements that may affect the wound or its surrounding area. 

5.No sun = no visible scar

Sun exposure increases the risk of hyperpigmentation in wounds, so avoid direct sun exposure as much as you can and always wear sunscreen and sun protection clothing.

6.Manual lymphatic drainage to heal fast

MLD has many benefits, some of them are: 

  • reduce swelling and edema, 
  • improve lymphatic and blood flow, 
  • reduce toxins and wastes, 
  • boost your immune system 
  • increase wound healing
  • and more…

7.Bonus… (I think my wound could be infected)

call your surgeon or GP if there is:

  •  too much pain in the area
  • signs of infection like redness, heat  or swelling
  • unexpected bleeding or pus
  • bad smell
  • your wound gets bigger or deeper
  • necrosis ( black area around the wound)

What is Ayurveda?

Ayurveda is a short term people use for ayurvedic medicine. It is one of the oldest holistic medicines that improves your general wellbeing. The term Ayurveda means ayur (life) and veda (science). In other words, Ayurveda translates the science of life.  On our previous blog you can find the most important characteristics of this holistic medicine.

According to Ayurveda, our universe consists of five elements:

  1. Ether
  2. Air
  3. Earth
  4. Fire
  5. Water

Ayurveda says that there are three principle bio-energies, these are the Doshas (vatta, pitta and kapha). They exist in all matter and every Dosha is also composed by different combinations of the five elements. So your body is composed by the combination of Doshas.

Ayurvedic medicine: Principles

Ayurveda is based on the belief that health and wellbeing depend on the balance between your mind, body, and spirit. In other words, when you are in harmony with the universe, you have good health; however, when something disrupts this balance, you get sick.

Causes of unbalanced

Among the things that can disrupt this balance are:

  • Genetics
  • Birth defects
  • Injuries
  • Climate and seasonal change
  • Age
  • Emotions.

How  know if your Doshas are unbalanced and how to balance them back.

1.Vata where the air and space elements dominate

Imbalance characteristics 

  • Constipation
  • Gas formation
  • Belly distension
  • Dehydration
  • Astringent taste in mouth
  • Dry and rough skin
  • Pains or general body-ache
  • Loss of strength and fatigue
  • Disturbed or lack of sleep (Insomnia)
  • Tremors and twitches
  • Feeling vertiginous
  • Sensitiveness to cold and desire for warmth
  • Feeling anxious, nervous or stressed
  • Desire of running away
  • Feeling confused, fearful & shaky

What you can do to balance Vata

Cold causes Vata to contract and tighten, in addition, restricts the free flow of movement that is so vital to your well-being. Vata needs warmth on all levels. In other words, you need to feel warmed by everything that surrounds you (your friends and family, the environment, what you eat, etc.). Some examples of what you can do are:

  • Stay warm.
  • Choose hot foods like meals with sriracha and use natural spices (cinnamon, curcuma, etc.).
  • Avoid extreme cold foods and frozen drinks.
  • Minimize consumption of raw foods, especially apples and members of the cabbage family.
  • Moderate your consumption of beans.
  • Make sure food is hot, moist and well lubricated, for example soup, hot drinks, and rice with a little oil or butter.
  • Choose sweet, sour, and salty flavours in your food.
  • Keep a regular routine.
  • Create a safe, calm, and secure environment (as much as you can).

Pitta where the fire element dominates

  • Lethargy
  • Decreased or no appetite at all
  • Nausea
  • Water retention
  • Congestion, mucous formation
  • Mouth secretions
  • Breathing difficulties

  • Avoid excessive heat, steam and moisture.

  • Get enough and good sources of carbohydrates to sustain and maintain adequate energy  levels.

7 food items you should eat to heal faster after surgery

After surgery, many people want to take soft chews multivitamin supplements and eat well to improve wound healing, reduce inflammation and all the recovery process by itself. A balanced diet gives you all the nutrients, vitamins and proteins that your body needs, so If your diet is healthy and balanced you don’t need supplements. On the other hand, if there is a nutrient deficiency your surgeon will prescribe you a supplement to fulfill your needs.

If you are a healthy person and want to heal faster from any surgery or illness just eat real food and avoid sugars and process food, simply like that!

Let’s have a look at which nutrients and vitamins play a role during your recovery process. 

What does my body need to heal faster?

Vitamin C

Vitamin C is a powerful antioxidant. It helps collagen formation.

It strengthens your connective tissue in your skin, muscle and blood vessels.  In addition, Vitamin C reduces inflammation, boosts your immune system and improves wound healing. You can find it in:

Vegetables:

Vitamin A is key and helps us with  many important and diverse biological functions. Some of them are: reproduction, embryological development, cellular differentiation, growth, immunity, and vision. Besides, when we talk about wound healing, it stimulates epidermal turnover, promotes collagen production, and accelerates the wound healing process.

Food sources of Vitamin A are:

  • Leafy green vegetables (kale, spinach, broccoli)
  • Mango, cantaloupe melon, watermelon, orange.
  • Yellow vegetables (carrots, sweet potatoes, pumpkin and other winter squash, summer squash)
  • Tomatoes
  • Red bell pepper
  • Cantaloupe
  • Beef liver
  • Fish oils
  • Milk
  • Eggs

But if you want to intake vitamin A supplements ask your surgeon first.

Zinc

Zinc is an essential micronutrient for your health. As its major role, it regulates every phase of the wound healing process; ranging from membrane repair, oxidative stress, coagulation, inflammation and immune defense, tissue re-epithelialization (resurfacing of a wound with new epithelium/skin or tissues), angiogenesis (process to form new blood vessels), to fibrosis/scar formation. When you are healthy, you get enough zinc through your diet, so there is no need for a supplement. Zinc is found in:

Bromelain

Bromelain is a mixture of enzymes that help to digest protein. In addition experts think that it helps to reduce swelling and inflammation after surgery. It is found in the fruit, skin and juice of the pineapple plant. However, if you want to take a supplement of it just ask your surgeon first.

Importance of good nutrition to heal

Protein malnutrition causes a decreased synthesis of protein and cell production; also, reduced immune function, and delayed progression of all phases of wound healing. In addition, vitamin and mineral deficiencies impair tissue synthesis, low energy production, reduced immune function, among others.

7 products to heal faster

1.Leafy green vegetables

They are high in vitamin C, manganese, magnesium, folate, and provitamin A, all of which are essential for immune function and overall health. They’re also rich in polyphenol antioxidants that have powerful anti-inflammatory and immune-supportive properties.

2.Eggs

Following surgery, your body needs more protein than the current recommended for tissue repairing.  Daily Allowance (RDA) of 0.36 grams per pound (0.8 grams per kg) of body weight.

After surgery, it is recommended to consume 0. 7–0.9 grams of protein per pound (1.5–2 grams per kg) of your body weight. That is to say, 105–135 grams for a 150-pound (68-kg) person.

3.Salmon

Protein, B vitamins, selenium, iron, zinc, and omega-3 fats key in wound healing.

4.Berries

They are full of antioxidants, are a good source of vitamin C. Berries have anti-inflammatory, antiviral, and immune-supporting effects.

5.Nuts and seeds

They are a good source of zinc, vitamin E, manganese, and magnesium. Vitamin E is an antioxidant that protects your body against cellular damage and boosts the immune system. Vitamin E improves the function of protective immune cells that help fight infection and disease.

6.Sweet potatoes

They are a healthy high carb source. Carbs provide energy to your cells (needed for healing) and also enzymes like hexokinase and citrate synthase, which improve wound repair.

7.Cruciferous veggies

These kinds of veggies have glucosinolates, then your body converts them into isothiocyanates. Isothiocyanates main functions are:  promote immune health by suppressing inflammation, activating immune defenses, and inducing death in infected cells.

Arnica Montana

Arnica Montana is a flower that is made into a soothing ointment, gel, cream, or pill that helps you heal with comfort after local trauma. 

Surgeons may recommend Arnica Montana (topical gel or cream) to reduce bruising and swelling after surgery, liposuction (5-7 days after surgery), certain facial procedures or injectable treatments (but only on certain areas). Never apply it directly to open wounds or your incisions. Use it to lightly massage the areas of bruising around the cheeks, mouth or lower eyelids. Besides, it has other uses like:
