Vascularized Lymph node transfer (VLNT) for Lymphoedema: Is it possible and works?

Persistent swelling? Lymphoedema is a chronic swelling condition, most seen in legs, arms or the nearest area where there is a lymphatic system dysfunction. It´s mainly caused  by lymph node removal or damage during cancer treatments or other types of surgeries; trauma or genetic. Lymphoedema treatment is based on hands-on treatments and lifestyle changes. But in recent years, surgeons have been performing what seems to be a promising surgical solution. It is lymph node transfer surgery, also known as vascularized lymph node transfer (VLNT) for lymphoedema. A surgical option that offers hope for many patients who thought their condition was irreversible. By now, you may wonder: Is it possible and works? Let’s have a closer look…

What is Vascularised Lymph Node Transfer (VLNT)?

Vascularised lymph node transfer (VLNT) is a microsurgical procedure performed by experienced surgeons who harvest healthy lymph nodes (from a donor site e.g. groin, abdomen, or neck) complete with their blood vessels, and transplant them into the area of the limb affected by lymphoedema. By connecting these nodes to local blood vessels, surgeons create new pathways for lymphatic fluid to drain, thereby restoring a more functional lymphatic operation.

Vascularised Lymph Node Transfer (VLNT) for Lymphoedema: How does it work?

The process involves the following steps:

  1. Donor site selection. Surgeons choose the best area with surplus (extra) lymph nodes; in order to minimize the risk of causing lymphedema at the donor site.
  2. They use reverse lymphatic mapping and imaging as help to ensure safety.
  3. Lymphatic nodes are harvested with their blood vessels.
  4. Transplantation. Specialised surgeons transplant, meticulously, the nodes and reconnect them to blood vessels using microscopes and fine sutures.
  5. Lymphangiogenesis. During the following weeks and months, the transplanted nodes encourage lymphangiogenesis, the growth of new lymphatic vessels, which can connect to the local circulation and start rerouting lymph fluid effectively to the nearest nodes. Here is where magic happens!

What does science say about VLNT?

Recent studies made in 89 patients – 88 female and 1 male-with different types of cancer (65 breast, 18 gynecologic, 2 melanoma and 4 other types of cancer) reported:

  • Limb volume (swelling) reduction. The first year, limb volume reduced 8%, and the second year, it reduced 20%-28% or more.
  • Less symptoms. Patients reported less swelling, heaviness, pain, and risk of complications such as cellulitis (skin infections).
  • Improved life quality. Many patients can reduce daily compression garments after successful surgery. It has to be always advised and supervised by their lymphoedema team (surgeon, lymphoedema therapist, etc.).
  • Restore immune function. The transplanted nodes help regain immune surveillance in the affected limb. In other words, restore lymphatic function to the area, reducing the frequency of infections.

Vascularized lymph node for lymphoedema: Is it safe?

Yes, it is. The reverse lymphatic mapping reduces  the risk of developing lymphoedema at the donor site. 

Are there any risk factors?

As with any surgery, risks include infection, scarring, or failure of the harvest nodes to survive However, major complications are rare, for that reason your surgery should be performed by experienced microsurgeons. 

Are there any side effects?

A case study of 89 patients after VLNT 14 reported complications like:

  • 1 hernia in the donor site
  • 1 episode of transient pancreatitis (pancreas inflammation-treated with medicines)
  • 2 cases of flap loss (transplanted piece of tissue loss ) 
  • 1 case of partial flap loss (just part of the tissue is lost)
  • 3 reported hematoma
  • 3 reported seroma
  • 2 postoperative infection
  • 1 wound dehiscence (wound open/tissue separation)

Vascularized Lymph Node for Lymphoedema:Are there more studies or trials?

Yes, there are, here some key Clinical Studies and Trials

1. Randomized Controlled Trial – Stage II Breast Cancer–Related Lymphedema

  • 36 women randomized: 18 underwent VLNT + physiotherapy/compression, and 18 had conservative therapy only
  • After 18 months: VLNT group had an average 57% volume reduction vs 18% in control
  • Infection episodes dropped significantly in VLNT group
  • Functional improvements and reduced heaviness/pain were reported
  • VLNT found to be more cost‑effective than conservative care alone

2. Large Prospective Series – Combined VLNT & Lymphovenous Bypass (LVB)

  • 328 patients treated; 67% underwent VLNT + LVB
  • Avg volume reductions: 21.4% at 1 year, 36.2% at 2 years, with sustained benefits up to 4 years
  • Quality-of-life scores (Lymphedema Life Impact Scale) improved significantly over time

3. Suction-Assisted Lipectomy (SAL) + VLNT

  • 94 patients with stage IIb–III lymphedema
  • Over ~3 years follow‑up: 60% reduction in lower-limb circumference and up to 81% in upper limb at various levels
  • Cellulitis episodes were significantly reduced 

4. Lymph Node Flap Transfer for Lower Limb Lymphedema

  • Retrospective review of 52 cases: average 46.3% volume reduction in unilateral mild-to-moderate cases
  • Better outcomes seen in less advanced presentations 

5. Safety & Complications – Autologous Lymph Node Transplant (ALNT)

  • Study of 26 patients (upper and lower limb): 38% had donor-site complications, including chronic lymphedema, lymphocele, pain
  • Highlights need for careful patient selection and surgical technique 

6. Emerging Techniques – VLNT + Nanofibrillar Collagen Scaffolds (NCS)

  • Observational comparison: VLNT alone vs VLNT combined with BioBridge® NCS
  • Early results suggest enhanced lymphangiogenesis and longer-lasting drainage pathways with the scaffold-assisted method 

How long is the recovery process?

Most patients stay in the hospital for several days or as suggested by their surgeons. 

When can I see the results?

  • Studies and clinical follow ups showed that most patients see full benefits of surgery up to two years afterwards. 
  • Remember that regular follow-up, physiotherapy (with your lymphoedema therapist), compression therapy, exercise, proper hydration and nutrition are essential during the recovery process and maintenance.

Who is a good candidate for VLNT or similar surgeries? When can I consider VLNT or similar surgeries?

  • This surgery is most effective for people with secondary lymphedema (often due to cancer treatments)
  • Patients who have not found lasting relief from conservative therapies. 
  • Lymphoedema carriers who get into an agreement with their surgeon and see VLNT or similar as the best treatment option.

To keep in mind: careful assessment and imaging guide patient selection (to see if you have donor sites or which one is the best for harvesting). These procedures are what ensure the best possible outcomes.

A final word

Vascularised lymph node transfer represents a new hope in the approach of lymphedema treatment and management. As it directly addresses the underlying lymphatic failure, it offers not just symptom relief but the opportunity for real, lasting results. Remember that the surgery by itself cannot do all the work, you should keep exercising, eating and hydrating properly, doing manual lymphatic drainage; and all together will give you long-lasting relief, improve your quality of life and, why not?, making you free from daily compression and discomfort.

Why is inflammation a risk factor for illness and chronic conditions?

Acute inflammation is the natural response from your body to a threat like a virus or infection; in order to fight and eliminate it. On the other hand, chronic inflammation can damage different tissues, cells or even change gene expressions (change your genes/mutate- e.g. cancer) in your body. In other words, nowadays, chronic inflammation is the main cause of autoimmune and chronic conditions.

How can I reduce inflammation and swelling naturally?

1. The hidden power of anti-inflammatory foods

A diet rich in anti-inflammatory foods can help calm your body’s inflammatory response and reduce inflammation. These are some of the ingredients you can introduce to your daily meals:

  • Leafy Greens: Spinach, kale, collards, and other green vegetables are loaded with antioxidants and nutrients that help fight inflammation.
  • Berries and Cherries: Blueberries, strawberries, and cherries are packed with antioxidants and have been shown to reduce swelling and pain.

  • Stay hydrated: Drinking enough water helps maintain fluid balance and may reduce swelling.
  • Elevate and rest swollen areas: For localised swelling, elevate the affected area above heart level and rest to promote fluid drainage.
  • Manual lymphatic drainage: This special massage technique reduces inflammation and swelling, reduces stress and anxiety while promoting the sensation of wellness. Always performed by a certified professional therapist.

4. Home remedies and supportive practices

  • Herbal teas: Green tea, ginger tea, and dandelion tea have anti-inflammatory and mild diuretic properties that can help reduce swelling.

In this blog, I’ll walk you through what menopause is, the symptoms you may experience, and the treatments that can help manage them during this transition. Let’s dive in!

What is menopause?

Menopause is a natural biological process that occurs when a woman hasn’t had a menstrual period for 12 consecutive months. It typically happens between the ages of 49 and 55, although it can begin earlier or later. Once your periods stop completely, you enter what’s known as post-menopause—the beginning of the next exciting chapter in your life!

In the past, society often viewed menopause as the end of a woman’s “youth,” simply because the natural reproductive process comes to a close. How wrong they were! Menopause is actually the beginning of a renewed woman, empowered to live life on her own terms. Yes, even motherhood is still possible—with a little help. It’s a time to embrace a fresh perspective, become wiser, bolder, and stronger than ever. That, my dear, is what menopause truly is—just another incredible cycle of our lives!

What are the symptoms of menopause?

Its symptoms result from a decrease in estrogen production in the ovaries and in the brain. The ovaries are responsible for approximately 75-80% of estrogen production, while the brain contributes around 20-25%, according to studies. As a result, the brain may trigger some of these symptoms. However, it’s essential to note that some women may experience few to no symptoms during this transition. Some of the most common symptoms include:

  • Hot flashes
  • Night sweats
  • Brain fog
  • Reduced muscle tone
  • Memory issues (forget thighs, dates, words, tasks, etc.)
  • Slow motion (you may move slowly)
  • Insomnia/Sleep disturbances
  • Mood swings
  • Skin, vaginal or eye dryness
  • Changes in libido
  • Tiredness

What is estrogen?

The ovaries, adrenal glands,  fat tissues and the brain produce estrogen. Both female and male bodies have this hormone, but females create more of it. Estrogen is fuel for women’s brains as testosterone is for men.

Estrogen is a group of hormones that play a crucial role in the development and regulation of different female functions, for example:

  • Development of the reproductive system. 
  • Regulation of  sexual characteristics.
  • Regulation of the Menstrual Cycle.
  • Help control the growth and shedding of the uterine lining.
  • Preparation for pregnancy.
  • Fetal development support.
  • Reduce bone loss by promoting bone density.
  • Mood regulation influence.
  • Cognitive function support.
  • Heart and blood vessels protection.
  • Others

Why can I have early end of menstrual period?

It can be due to different factors, some of them are:

  • Trauma (ovarian or uterus removal surgery)
  • Genetics
  • Hormonal levels (how much estrogen you are producing )

How can I deal with symptoms during menopause?

Swollen lymph nodes? Mpox symptoms and progression

Monkeypox is endemic in parts of Central and West Africa, but recent outbreaks in regions where the virus is not typically found have sparked global concern. These outbreaks have prompted increased surveillance, vaccination efforts, and public health measures to control the virus’s spread.

From 2023 to 2024, there were 160 new cases of Mpox reported: 153 in England, 3 in Scotland, 1 in Wales, and 3 in Northern Ireland. Mpox, caused by the monkeypox virus, is related to the smallpox virus. A notable symptom of Mpox is lymphadenopathy (swollen lymph nodes), which is a response of the immune system as it fights the infection. This swelling can occur in various regions, including the neck, armpits, and groin, and may affect lymph nodes on one or both sides of the body.

How can I get the Mpox?

From animals to Human :

  • It is spread by rodents: rats, squirrels and mice.
  • Infected animal bite.
  • By touching  blood, body fluids, spots, blisters, scabs, skin or fur.
  • Ingesting poorly cooked meat from an infected animal.

From person-to-person: 

  • Touching clothing, bedding or towels used by someone with the monkeypox rash.
  • The coughs or sneezes of a person with the monkeypox rash.
  • Direct contact with monkeypox skin lesions or scabs.

What are the symptoms of Monkeypox? 

First period

The incubation period (before first symptoms) is usually 7−14 days, but can range from 5−21 days.

  • High temperature
  • Headache
  • Muscle pain
  • Back pain
  • Shivering (chills)
  • Exhaustion
  • Swollen lymph nodes

Second period

Lesions progress through the following stages before falling off:

  • Macules. It is a flat, distinct, discolored area of skin (less than 1cm wide). 
  • Papules. Raised area of skin tissue ( less than 1 cm around). 
  • Vesicles. Small fluid-filled sacs.
  • Pustules. Bulging patch of skin that’s full of a yellowish fluid (pus).
  • Scabs. Patches of dry, tough skin that form over a wound during the process of healing.

Monkeypox typically lasts 2−4 weeks. Currently, the UK Health Security Agency (UKHSA) states that the risk of Mpox to the general population in the UK is considered low, although health authorities remain vigilant for potential new cases and variants.

How can I avoid getting Monkeypox?

Here, some tips to keep you out of this virus:

  • Regular hand wash with soap and water or use an alcohol-based hand sanitiser.
  • Only consume meat that is well-cooked.
  • Don’t be close to infected animals or people.

Lymphatic Function in Schizophrenia: Understanding this mental health condition

There is available evidence suggesting that abnormalities in the lymphatic system function may be associated with schizophrenia, specially the vessels located in your brain (the ones that drain wastes from your brain). In this blog you can find what schizophrenia is, its main symptoms, how the misfunction of the lymphatic system could be related to this mental health condition and what lifestyle changes you can introduce in order to improve its symptoms. Let’s start with the basics…

What is schizophrenia?

Schizophrenia is a serious mental health condition that affects how people think, feel and behave. It is due to the mix of hallucinations (see and hear things or voices that are not perceived by others), delusions (firm beliefs about things that are not true), and disorganized thinking and behavior. For people with schizophrenia daily life can be pretty hard because they have lost touch with reality. 

Many people with schizophrenia don’t know they have a mental health condition, so they don’t  know they need treatment. For that reason, it’s important that relatives (family, friends, coworkers, etc.) pay attention to the symptoms in order to help  them get a diagnosis and proper treatment.

What are the main symptoms of schizophrenia?

Symptoms of schizophrenia produce changes in how people think, feel and behave. The most common are:

Delusions.  To believe in things that aren’t real or true. For example, being harmed or attacked when they aren’t. They may think they’re famous, but they are not, etc. Most people with schizophrenia have delusions.

Hallucinations. It’s when you see or hear things that other people don’t observe. For people with schizophrenia, these things seem real. Hallucinations can occur with any of the senses, but hearing voices is most common.

Disorganized speech and thinking. It’s hard for people with schizophrenia to talk to other people because their disorganized thinking and misconception of reality makes them have a disorganized and unrealistic speech. In other words, when you ask them something, they will answer something completely different or not related to your inquiry.

Unusual motor behavior. Some examples are:

  •  childlike silliness
  •  being agitated for no reason
  • lack of focus on specific goals (unable to complete tasks)
  • may not want to follow instructions
  • inappropriate social movements
  • unusual movements
  • no movement or response at all

Behavioral changes. People with schizophrenia may not be able to function in the way they could before their illness started. For example:

  • don’t take a baths
  • do not make eye contact 
  • cannot show emotions 
  • speak in a monotone voice 
  • not be able to feel pleasure
  • lose interest in everyday or social activities
  • unable to make future plans

How is the lymphatic system related to schizophrenia?

The meningeal lymphatic vessels (lymphatic vessels in your brain) remove wastes and toxins from your brain to keep it healthy and working properly. Studies showed that when your meningeal lymphatic vessel don´t work properly, it produces cognitive impairment (changes in the way your brain perceives and processes information) that can result in behavioral changes (like those seen in schizophrenia). There is also evidence of changes in the functional activity of the lymphoid tissue of the inguinal lymph nodes in patients with schizophrenia. In other words,  when the lymphatic system doesn’t work properly, your body, especially your brain, is unable to clean by itself and can not regulate the cognitive and immune function properly.

In short, accumulated fluids and wastes produce or exacerbate these behavioral and cognitive changes seen in schizophrenia. However, further research is still needed to fully elucidate the role of the lymphatic system in the pathophysiology of this mental health condition.

What lifestyle changes can enhance lymphatic and immune function in schizophrenia?

There are some lifestyle changes that could enhance lymphatic function in individuals with schizophrenia, however, these tips don’t replace professional treatment (medication and talk therapy), they can be part of an holistic approach, here some of them:

  1. Exercise: Regular physical activity improves brain health and circulation promoting waste clearance and cognitive function. 
  2. Healthy Sleep Habits: Adequate and quality sleep is crucial for glymphatic function (the glymphatic system in charge of cleaning your brain while you sleep). So sleep well, to feel and function well.
  3. Dietary Modifications: A balanced and nutritious diet rich in antioxidants and anti-inflammatory foods may support brain health and potentially enhance lymphatic function. 
  4. Lymphatic drainage massage: MLD boosts your lymphatic function, improves circulation, speeds up waste explanation, boosts your immune system, reduces stress and anxiety, etc. 
  5. Stress Management: Chronic stress can negatively impact brain health and cognitive function. Practicing stress-reducing techniques such as mindfulness, meditation, specialised massage techniques or yoga may help lower stress levels. 
  6. Hydration: Staying well-hydrated is essential for overall brain health and function. Drinking an adequate amount of water throughout the day will support the glymphatic system in clearing waste products from the brain.

By incorporating these lifestyle changes to their mental health treatment (medication and talk therapy, etc.), people with schizophrenia may potentially enhance glymphatic, lymphatic and immune function, which could contribute to improved cognitive function and overall brain health.

How long does it take to recover from cosmetic surgery?

The recovery time from cosmetic surgery varies depending on the specific procedure performed, individual factors such as overall health and the body’s natural healing process, as well as the extent of the surgery. Many surgeons promote the notion that post-surgery recovery lasts two to three weeks and assures patients of their ability to return to work within this timeframe. However, my observations in practice diverge significantly from this narrative. Even minor cosmetic surgeries necessitate a healing period exceeding three weeks. Patients, usually, start feeling normal around the six-week mark, notice some improvements by three months, and often only witness the final results after six months.

What  is the hardest plastic surgery to recover from?

Determining the most painful cosmetic surgery recovery can be subjective and dependent on individual pain tolerance, surgical techniques, and the extent of the procedure. However, certain cosmetic surgeries are often associated with higher levels of discomfort during recovery due to the nature of the surgery and the areas of the body involved. 

From my practice experience, I’ve put together a list of the most demanding plastic surgeries in terms of recovery that I handle.

Multiple Procedures: Undergoing multiple plastic surgery procedures simultaneously, such as a mommy makeover combining breast augmentation, tummy tuck,  liposuction and BBL, can result in a more challenging recovery due to the cumulative effects on the body. 

Abdominoplasty (Tummy Tuck): Following the procedure, patients often experience notable discomfort, tightness, and soreness in the abdominal region during the early stages of recovery.  

Breast Augmentation with Implants: In the initial week following the procedure, patients commonly encounter sensations of discomfort, tightness, and pressure in the chest area as the tissues adapt to the implants. 

Liposuction: During the recovery period, patients often encounter soreness, swelling, and bruising in the treated areas, leading to discomfort. However, a primary challenge in recovery arises from excessive fibrosis, or the buildup of connective tissue, which can be painful and contribute to irregularities in the skin’s texture. Early intervention by a therapist skilled in managing fibrosis is crucial for addressing these irregularities effectively and minimizing potential complications.

Facelift:  This procedure can lead to tightness, swelling, bruising, and discomfort in the face and neck during recovery. One notable challenge associated with this surgery is the persistent sensation of skin pulling and prolonged inflammation compared to other procedures.

Moreover, because facial features are prominently visible, any inflammation or irregularity becomes more apparent and can be particularly frustrating for patients.

Rhinoplasty (Nose Surgery): Patients commonly experience  discomfort, particularly in the nasal region, including sensations of blockages and sinus pressure, in the initial stages of recovery. 

Body Lift Surgery: Often leads to discomfort, swelling, and restricted mobility throughout the recovery period. Patients frequently report sensations of tightness, numbness and soreness in the treated areas. A significant concern I encounter with my patients is their insufficient understanding of wound care, resulting in frequent infections.

What is the riskiest cosmetic surgery ?

The riskiest cosmetic surgery varies depending on several factors, including your individual’s health status, the surgeon’s skill and experience, and the specific procedure performed. However, certain cosmetic surgeries are generally associated with higher risks and potential complications compared to others. The procedures that are considered the riskiest are “Brazilian Butt Lift” (BBL) and Tummy tuck . 

Brazilian Butt Lift (BBL)

A BBL procedure typically entails removing fat from one part of the body via liposuction and injecting it into the buttocks to achieve a fuller appearance.

Risks and Complications:

  1. Fat Embolism
  2. Infection
  3. Asymmetry and Irregularities
  4. Skin Necrosis
  5. Pulmonary Complications
Tummy tuck  or abdominoplasty 

Moreover, in my experience, tummy tuck procedures performed in Turkey exhibit a heightened risk of complications. Out of every 10 tummy tuck patients who seek treatment at my practice, I find it necessary to refer one to the A & E. These referrals typically stem from infected and open wounds, symptoms of sepsis and occasionally, the need for emergency surgery.

What is the most painful surgery to recover from?

Determining the most painful cosmetic surgery to recover from can vary depending on individual pain thresholds and the specific procedures involved. However, my patients express more post- operative discomfort with certain cosmetic surgeries such as:

Rhinoplasty, commonly known as a nose job, can be particularly painful due to the intricate nature of reshaping the nasal structure. Patients may experience significant swelling, bruising, and nasal congestion during the recovery period, leading to discomfort.

Abdominoplasty, or tummy tuck surgery , can be quite painful due to the extensive tissue manipulation and muscle tightening involved. Patients often report tightness, soreness, and discomfort in the abdominal area during the initial stages of recovery.

Breast augmentation with implants, which involves placing implants to enhance breast size and shape, can also cause discomfort, especially in the chest and upper back area, as the tissues adjust to the implants.

How long are you off work after plastic or cosmetic surgery?

Determining the duration of time off work after plastic or cosmetic surgery depends on various factors, including the type of procedure, individual healing capacity, and the physical demands of your job. Typically, patients may need to take anywhere from a few days to several weeks off work to ensure a smooth recovery process.

For minor cosmetic procedures such as chin-lipo patients often return to work within a day or two. 

More extensive surgeries such as breast augmentation,  liposuction, abdominoplasty (tummy tuck), or facelifts may require a longer recovery period. Patients typically take about two to four weeks off work to allow for initial healing and to manage any discomfort or swelling.

For physically demanding jobs or those involving strenuous activity, patients may need to take additional time off to avoid compromising their recovery or risking injury.

“Bye, bye Fibrosis”: 7 treatments that help to break down fibrosis after aesthetic surgery

Breaking down fibrosis  after aesthetic surgery (like liposuction, BBL or Tummy tuck) requires tailored treatments to alleviate discomfort, reduce inflammation, pain and address tissue irregularities; in order to get the best results. In this blog, we´ll explore seven methos that will make you say “bye” to this uncomfortable issue. But first, let´s start with the basics…

Fibrosis after liposuction, BBL or  tummy tuck, is part of the healing process and occurs when your body develops excessive fibers of fibroblast ( made of collagen) creating irregularities in  the tissue.  Factors such as, lack of compression, genetics, individual healing, used surgical techniques can influence the likelihood and severity of fibrosis.

Patients may experience fibrosis between 1 to 12 weeks after surgery, with some cases presenting later. Symptoms of fibrosis include:

  • Local pain 
  • hard lumps
  • skin discoloration 
  • irregularities, retractions 
  • skin adhesions
  • limited movement (lack of  tissue flexibility) 
  • complex scar tissue formation (overproduction of scar tissue)
  • Seroma (accumulated fluid pocket)

To get the best outcomes, the treatment should be  tailored to each patient depending on the stage of the healing and the characteristics of their fibrosis at the moment of the consultation. 

 Break down Fibrosis: How do you treat fibrosis after liposuction, BBL or Tummy tuck? 

There are 7 proven methos to break down fibrosis after aesthetic surgery, let´s know them!

1. Massage modalities 

  • Manual lymphatic drainage

This  massage technique specializes and helps to stimulate lymphatic flow and reduce swelling. It may also break down fibrosis (fibrous tissue) and improve the smoothness of the skin when done properly. Manual lymphatic drainage is essential in the first week of your recovery. The quicker your body absorbs or eliminates the excess of  fluids, the easier your skin can attach  and the body will create less fibrosis. 

  •  Scar tissue/ fibrosis massage

This massage technique targets  deep scar tissue fibers. The massage can help break down fibrous adhesions and improve the flexibility and mobility  in the surgery area. Scar tissue massage isn’t a deep tissue massage. Your therapist must perform this massage carefully and gently in order to avoid further damage and irritation of the tissue.  For that reason, you should look for a qualified healthcare professional, such as a physiotherapist  or  massage therapist, who has experience in working with scar tissue.

2. Adequate compression 

  • Use of Faja or Garment 

A compression garment (faja) can help to minimize swelling and related discomfort after surgery. Maintaining constant pressure on the surgical site will prevent excess build-up of subcutaneous fluid  or swelling, and encourage your body to reabsorb the fluid that it has been accumulating.

Wear your compression garment  24/7, between 4 to 6 weeks  (with the exception of showering or during your post op treatment ). The exact timeframe will vary depending on your body healing process, the type of procedure you had and any additional factors determined by your plastic surgeon or  therapist. After their approval you can wear the garment for 12 hours per day (day or night, that it´s up to you) for several weeks (until your fibrosis breaks down).

Take into account that if your faja is too tight, it will reduce blood  and oxygen flow which increases fibrosis, so wear the appropriate compression garment. If you are not sure about the right compression, you can contact me, I´ll be glad to help!

  •  Additional padding: 360 foam/ lipo foam 

Compression foam plays a vital role in the post-op recovery  after cosmetic surgery. Unlike traditional pressure garments,  foam provides even pressure over the treated area, leading to even healing and better results. 

Experts recommend 360 foam when you have fibrosis in the back and abdomen (e.g. after lipo 360). Some of the reasons I recommend it, are: it doesn´t move from place; It gives you uniform compression in abdomen and back. Besides, it doesn´t allow the formation of  skin irregularities, thanks to its structure. 360 foam is a hole unit that uses a Velcro fastening system in  the back. In addition, it adds extra compression (you can tight it as much as you want). If you can find it, it will be the best option to compress large areas such as abdomen and back.

Other type of foam I recommend is a lipo foam compression sheet.  Experts recommend lipo foam for the abdominal area after liposuction if 360 foam isn’t available.  Lipo foam works wonders in areas like the chin, arms, legs and upper back. 

Both 360 foam and Lipo foam help to:

  • form a barrier between the garment and the skin,
  •  prevent  sink burns, 
  • avoid discomfort, marking, creasing and digging of the faja,
  • apply uniform pressure on the skin reducing swelling and bruises,
  • improve body shape generating a molding effect,
  • provide even distribution of the fat cells, as a result, you will have better body shape and appearance.

3. Equipment

  •  Ultrasound Therapy: 

Ultrasound therapy uses high-frequency sound waves to penetrate deep into tissues, generating heat and promoting circulation. In the management of fibrosis, ultrasound therapy helps to:

  • reduce inflammation, 
  • break down scar tissue, by promoting tissue relaxation and increasing the flexibility of collagen fibers, 
  • aid in softening and reducing the density of the scar tissue, 
  • improve range of motion, 
  • reduce pain and stiffness. 

It’s important that  a qualified healthcare professional, such as a physical therapist or physician,  treats you with ultrasound because these kind of specialist can assess the tissue and tailor the treatment according to your needs.

  •  Radiofrequency (RF) Therapy

 Radiofrequency(RF)  therapy uses energy to heat the skin and underlying tissues, stimulating collagen production, and promoting skin tightening. This can be beneficial in reducing the appearance of fibrosis. South American surgeons recommend Monopolar radiofrequency to treat fibrosis. However, they use it only at the end of the proliferative phase of the healing. 

  •  Cavitation

Cavitation ultrasound uses focused ultrasound waves.  Experts use it to soften the trapped adipose (fat) within the fibrotic tissue beneath the skin. This can be beneficial in reducing the appearance of fibrosis. Cavitation is only beneficial for the patient if introduced at the right stage of the healing. On the other hand, if used for prolonged time and the wrong settings, it could increase the production of fibrosis due to the nature of the ultrasound waves.   

4. Laser Therapy

Laser treatments stimulate collagen production and improve skin texture.  Experts use different types of lasers according to the specific characteristics of the fibrosis. I haven’t used lessers yet in my post surgery treatments, so i can’t comment in this machine, but i know some surgeons recommended. 

5.Exercise 

Gentle exercises, including stretching and targeted movements, I always recommend to improve tissue flexibility and break down fibrous adhesions. 

6.Medications – Corticosteroid Injections

In severe cases, corticosteroid injections may be considered to reduce inflammation and soften fibrous tissue. This is typically used in cases where the fibrosis is more pronounced and  tissue didn’t respond to other treatments. Your surgeon is the only professional who can prescribe them.

7.Surgical Revision

In severe cases, surgical revision may be considered to address persistent irregularities. This involves additional surgery to remove or reposition excess fat, scar tissue  and reshape the treated area. Your surgeon will recommend this procedure just in extreme cases or when other treatments have failed.

 Please make sure you find a professional specialised in post-surgery after care as not all the equipment and techniques listed above can be used in each patient. In addition, not all the equipment can be used in the same appointment. A  well experienced professional will be able to combine the equipment and techniques based on your particular needs. 

Break down fibrosis: in short…

It is crucial to seek professional guidance from a specialized post-surgery  after care provider who can tailor a treatment plan based on individual needs. Early intervention and adherence to recommended protocol can effectively manage fibrosis post-surgery. Approximately 80% of fibrosis cases can be successfully treated with manual techniques and proper compression methods when overseen by a skilled post-surgery specialist.

Lymphatic Drainage after Breast augmentation or reduction surgery

What is mammoplasty (breast augmentation)?

Mammoplasty or a breast augmentation procedure is a surgery in which implants are placed under your breast tissue or chest muscles.

Why is mammoplasty performed?

Some of the most common reasons are:

  • improve your self-confidence
  • rebuild the breast after a previous condition, for example breast cancer
  • help with asymmetry (when each breast has a different and notable size)

Mammoplasty: Types of implants

There are two types of implants, your surgeon will advise you what is best according to your needs.

  • Saline are inserted empty and then filled with sterile salt water once they’re in place. 
  • Silicone are pre-filled with silicone gel.

Breast augmentation: What next?

After the procedure you can expect:

  • Soreness and swelling for a few weeks.
  • Bruising (in some cases).
  • Scars will fade, but not disappear completely.
  • Wear a  sports bra for extra support and positioning of the breast implants.
  • Avoid activities that raise your blood pressure.
  • Pain medication will be prescribed by your surgeon.
  • Your breast will be pretty sensitive.
  • Difficulty while lifting your arms (pain /stiffness).
  • Avoid overhead lifting movements because your implants can move.
  • Your surgeon will give you instructions to return to your daily activities.

Start as soon as possible (with your surgeon authorization) your post-op treatment in order to speed up your recovery process, be more comfortable while healing and get better results.

Breast reduction = reduction mammaplasty

What is mammaplasty?

It is a surgery that removes fat, breast tissue and skin from the breasts, in order to reduce their size.

Why is breast reduction performed?

Mammaplasty can help people with:

  •  large breasts to ease discomfort and improve appearance.
  •  to improve your self-esteem and the ability to take part in physical activities (like jumping, running, etc.).
  • chronic back, neck and shoulder pain due to breast size.
  • shoulder grooves from bra straps.
  • chronic rash or skin irritation under the breasts.
  • nerve pain.

What are  the risk of Breast surgery (augmentation or reduction)?

  • Fibrosis
  • Breast pain
  • Infection
  • Changes in nipple position
  • Breast sensation changes
  • Implant position changes/ implant displacement
  • Allergic reactions
  • Implant leakage or rupture
  • Noticeable scaring
  • Implant rejection
  • Asymmetry
  • Capsular contractures (scar tissue build up around the implant)

Some of these complications may need further surgery to correct them. Other patients may need their implants to be removed or replaced. However a post-surgery treatment (lymphatic drainage, etc.) can help you reduce these risks and get faster and better outcomes.

Warning sings of breast augmentation or reduction 

Visit your surgeon if you notice:

  • warmth and redness in your breast (sign of infection)
  • fever (sign of infection)
  • noticeable infection
  • shortness of breath

Dry brushing: From ayurveda medicine to your home

Dry Brushing is turning into a trend nowadays. However, there’s still a lot to learn about this ancient technique and its multiple benefits. For that reason, I wrote this blog to answer common questions our patients ask us about this technique.

What is Dry Brushing?

Dry Brushing is an Ayurvedic skin massage and exfoliation technique.  It’s an ancient self-care treatment that uses a coarse natural-fiber bristle brush to remove dead cells, massage your skin and improve circulation. Dry brush has been used for thousands of years  by Egyptians, Greeks, Indian and Chinese cultures.

 In Ayurvedic medicine, it is also known as Sanskrit :garshana (or “friction by rubbing”). It promotes the healthy and balanced fluid flow in your whole body.

 What does dry brush do and its benefits?

First of all, you should know that well performed dry brushing functions both on and under the skin’s surface. As its name says, it should be performed on dry skin, We recommend doing it before showering or bathing. Let’s have a look at their benefits: 

 1. Boosts Circulation

Dry skin brushing stimulates blood flow to the skin, improving blood circulation, helping the transportation of oxygen and nutrients to the tissues to promote your overall health.

2.Improve cellulite appearance 

As it increases blood circulation, it  might plump dimples in the skin, reducing  the appearance of cellulite because the skin texture is improved. The brushing needs to be done on a regular basis to see differences on the skin. Also, you need to have a healthy diet and regular exercise for long lasting results.

3.Skin exfoliation

Dry brushing sweeps off dead cells  and at the same time stimulates the production of  healthy cells to replace the old ones. In addition, it  leaves the skin with a lighter and smoother look.

4.Boosts lymphatic flow and works as a body detox

Dry brushing movements stimulate the lymphatic system, increasing lymph flow and circulation, as a result, wastes  and toxins from your body will be removed.   

5.Increases energy levels

This technique stimulates the nerve endings in your skin. And, as it increases circulation you may feel energised and refreshed. 

How many times a week should you dry brush?

 We recommend dry skin brushing 2 -3 times per week if you do it yourself. On the other hand, if you go to a professional, we recommend every two weeks.

How should dry brushing be performed? 

First of all, you need a  brush with coarse, natural-fiber bristles (long or short  handle-what works best for you).  Make sure the fibers of the brush are firm to perform the technique properly.  Use circulatory movements always towards the heart. 

  •  Start at your feet and move up.
  • Brush your skin using circular, clockwise motions or movements that feel comfortable for you.
  • Use light and firm pressure to avoid skin irritation. 
  • You can brush your entire body if you like, just avoid irritated areas and always start bottom up.
  • Brush the skin1 to 3 mins  until you see a light pink color on the surface of the skin. Never brush too hard until the skin is red. 
  • After it, take a shower to help remove the dry skin.
  • After shower don’t forget to moister your skin.

Keep in mind…

As your pores will be unclogged by dry brushing, your skin will need more moisture. Ayurvedic medicine texts say that this kind of technique is best followed by an oil massage in order to increase circulation and detoxification. Pairing the techniques is a highly effective formula for smooth, hydrated, and glowy skin.

What are the cons of dry brush?

  • Be careful while brushing too intensely or frequently, it may cause microtears in the skin that can become irritated, inflamed, and infected.
  • Avoid this technique if you have skin inflammatory conditions like psoriasis or eczema, open wounds or sunburned skin.

What about Ayurveda treatment?

Dry brushing is often a component of pancha karma -five actions- it is a traditional Ayurvedic cleansing treatment. Ayurveda practitioners believe this routine allows the body to get rid of accumulated waste, helping clear blockages in various systems, including the circulatory, nervous, and digestive systems.

Don´t forget to give proper maintenance to your dry brush

Here some steps to keep it clean and ready to use:

  • rinse your brush after you have completed your brushing routine;
  • dry it in an open, sunny area to prevent mildew;
  • wash your brush once a week using soap and water;
  • avoid sharing your brush with anyone, in order to prevent the risk for infection.

What is cranial osteopathy?
